General Rule: We respect the importance of keeping your health information private. It is our legal obligation to give you notices of our privacy practice. We can use your health information in or outside of our office without your written permission, only for purposes of treatment or other healthcare operations and support. In some limited and specific situations, the law allows or requires us to disclose your health information without written authorization.

How We Collect Information about You: SoundCure collects data through its SoundCure Serenade Tinnitus Treatment System as part of the evaluation necessary to program the Serenade Patient Device.

What We Do Not Do With Your Information: Information about you and/or your medical conditions and care is held in the strictest confidence and is not shared with anyone without written consent from you.
We do not give out, exchange, barter, rent, sell, lend, or disseminate any information about patients who use our product that is considered patient confidential, is restricted by law, or has been specifically restricted by a patient in a signed HIPAA consent form.

How We Do Use Your Information:
Information collected is used by your health care provider to assist in your tinnitus treatment. SoundCure may use your device programing data anonymously in aggregate for clinical evaluation, statistical analysis, or other scientific analysis.

Information We Do Not Collect: We do not collect your social security number or any other identifying information other than your name, address, age, and gender, and programing results.

Limited Right to Use Non-Identifying Personal Information:
We reserve the right to use non-identifying, anonymous information about patients for clinical evaluation, statistical analysis, or other scientific analysis.
No identifying information (addresses, phone numbers, contact information, last names or uniquely identifiable names) will be used without client’s express advance permission; anonymous information use is without compensation.

Your Rights Regarding Your Health Information: The law gives you many rights regarding your health information.

You can ask us to restrict our uses & disclosures for purposes of treatment.
You can ask us to communicate with you in a confidential way.
You can ask to see or to get photocopies of your health information.
You can ask us to amend your health information if you think that it is incorrect or incomplete.
You can get a list of the disclosures that we have made of your health information within the past six years.

Our Notice of Privacy Practices: By law, we must abide by the terms of this Notice of Privacy Practices until we choose to change it. We reserve the right to change this notice at any time as allowed by law.
